DR SHARON CRICHLOW

TERMS & CONDITIONS

New and follow up fees are made available on request and prices for any additional procedures will be quoted in full at the time of booking.

Self-funding Patients

Charges

The charge for your initial and follow up consultations are available on demand and are subject to change.

Please note that if you are officially discharged, are being seen again after a period of more than one year, or you have a new presentation with a different condition, you will need to pay the new consultation fee again. This applies to any one of the above.

The fee(s) for any procedure(s) required is (are) separate from the consultation fee and will be outlined at your consultation.

Fees for blood tests, the analysis of any skin swabs, and histology of any surgical procedures will be quoted before the test or procedure is done so that you are always aware of any expected charges. You WILL be charged by any pharmacy for prescriptions issued by the clinic.

Payment Arrangements

A deposit of at least £100 is necessary in order to secure the booking.

We reserve the right to cancel any appointment if this deposit remains unpaid 48 hours prior to the scheduled appointment date. If your appointment is cancelled due to non-payment of the deposit and you wish to be reinstated, please contact the clinic.

For procedures costing £400 or more, half the cost of the procedure will be taken in order to secure the booking. Usually this will be paid at the time of the booking, but where it is not, we reserve the right to cancel the appointment if it remains unpaid 48 hours before the appointment date.

The remainder of the payment will be taken in clinic immediately following the consultation or procedure.

The deposit can be reimbursed immediately following the appointment if you are discharged or can be held as a deposit for future consultations or procedures.

Failure to Attend / Cancel Within Notice Period

In the event of non-attendance or cancellation within 48 hours of the appointment, the £100 deposit is non-refundable.

Insured Patients

Charges

Fees for your initial and follow up consultations and for skin procedures have been agreed with your private medical insurance provider.

Please note that if you are officially discharged, are being seen again after a period of more than one year, or you have a new presentation with a different condition, your insurer will be invoiced a new consultation fee again. This applies to any one of the above.

Fees for blood tests, the analysis of any skin swabs, cryotherapy, and procedures will be charged in addition to the above by the clinic. You will need to pay for pharmacy prescriptions separately to the relevant pharmacy.

Payment Arrangements

Dr Crichlow will invoice your medical insurance provider directly for payment.

You acknowledge and agree that having the benefit of private medical insurance does not in any way absolve or diminish your responsibility to meet the fees payable in full. It is your responsibility to ensure that you have authorisation from your private medical insurance provider before obtaining the relevant health services.

In the event that your private medical insurance provider does not pay your fees, you undertake to pay those fees in full. Payment options and details will be sent by invoice.

Late Payment

If fees are not paid within 28 days of the invoice date, you agree to pay interest at a rate of 5% above the Bank of England base rate until the invoice has been paid in full.

Failure to settle your account will result in your details being forwarded to a debt collection company, which may result in additional fees being charged. You will not be able to book another appointment if payments remain outstanding.

Failure to Attend / Cancel Within Notice Period

Failure to attend an appointment without 48 hours’ cancellation notice will incur a fee of £100.

Please note that this fee is not covered by your insurance, and you as the patient will be liable for this charge.

Failure to pay will result in referral to a debt collection agency and the inability to book further appointments.

Prescription Charges

Please make every attempt to top up your prescriptions at booked appointments.

Prescriptions between appointments will only be issued if you have been reviewed within the last six months and will attract an administration fee of £25.

Prescriptions will be issued via an online pharmacy (usually Pharmacierge or another online pharmacy) and will not be posted. You will still need to pay for the medication itself.

After six months, under all circumstances, you will need to book a review appointment in order to obtain a repeat prescription. This can sometimes be carried out virtually depending on individual circumstances.

This policy has been introduced to limit the increasing volume of prescription requests between appointments.

Fire Safety

Complete Dermatology is located on the first floor of 5 Kinsbourne Court. Although there is a lift for ease of access, this is disabled in the event of a fire.

We therefore regret that we are unable to see or treat patients who are wheelchair bound or otherwise unable to mobilise without assistance from other persons. This is in line with our Fire Safety Policy, as we do not currently have sufficiently robust systems in place to enable timely evacuation of such patients should the need arise.
